For the transcription of the birth entry it is indispensible to have the presence of the parents (if they are married, the presence of one shall suffice) and of two witnesses in possession of valid identification documents.
For the transcription of the birth certificate, they must be in possession of the following documents:

1. A duly filled out form.
2. Full Copy of Birth Entry, Full Birth Certificate, Identity Card or Passport
3. Consular registration of the parents (Angolan citizen)
4. Residence Permit for Switzerland
5. Original Birth Certificate of the applicant (minor)
6. Fees : CHF 60.-

N.B.: The full copy of the birth entry or the birth certificate must be duly authenticated by the Ministry of External Relations of Angola (validity of six months or less).
Important: All documents in a foreign language (except passport, international certificate of vaccination and proof of flight and hotel bookings) must be previously translated into Portuguese by an official translator and authenticated by the local authorities (canton or federal notary/chancellery).
