Visa for medical treatment

For a Visa you should provide the following documents :

1. Letter of the petitioner directed to the Embassy
2. Completely filled out form
3. Health Medical Certificate
4. Proof of the medical consultation
5. Health institution where the petitioner will be intern
6. Police Clearance authenticated that applicant has no pending criminal proceedings
7. Proof of means of subsistence while in Angola
8. Declaration of commitment according to the Angolan law
9. Passport and copy of the main used pages
10. Three additional passport size pictures
11. Payment of the migratory act (photocopy)

Note: All the documents in foreign language (except the passport, the international certificate of vaccines and supporting documents of reserves of flight and hotel), must be translated into Portuguese by an official translator and legalized by the local authorities (notary/chancellery of the canton or federal)
